The Trevi Fountain was easily the most heavily trafficked attraction we visited in Rome other than the Colosseum. It was also #1 on Ryan's list of things he wanted to do, so we structured a big part of our second day around making sure we got there during the daylight hours. Honestly, the crowds were a bit overwhelming for me during the afternoon, but the fountain is stunning and well worth moving around your schedule to visit.
It's walking distance from the Spanish Steps, so I'd say it's a good idea to do the main things in that part of town in one chunk (you can also walk to the Pantheon from here, but I'll have more on that later). If you're up for it, try going during the day and again near dusk or during the evening. For one, it is WAY less crowded at night, and it's quite beautiful in a totally different way than during the day. For two, there are a lot of great bars and restaurants in the area, so you can dip in for some yummy dinner after a night visit to the fountain!
